Binocular balancing exists to solve one problem: a refraction measured one eye at a time is not necessarily the refraction the patient uses with both eyes open. Candidates who learn the techniques as a sequence of steps tend to struggle when a patient does not respond as the textbook says. Candidates who understand what each technique is manipulating do not. What binocular balancing involves
The methods you are expected to know are Humphriss, duochrome and blur-back, along with recording the final acuity in LogMAR. Each approaches the same problem differently. Humphriss keeps both eyes open and uses a fogging lens to suppress one while the other is refined. Duochrome uses colour rather than blur. Blur-back works by fogging both eyes and reducing symmetrically. Knowing which to reach for, and when one of them will not work, is more useful than being able to perform all three mechanically.
The optics to be fluent in
Duochrome depends on the eye's longitudinal chromatic aberration, which is why red and green targets focus at slightly different planes, and understanding that also tells you when the test becomes unreliable. Humphriss depends on the fogging lens producing enough blur to suppress central vision without breaking fusion. Be clear about what accommodation does during each technique, since accommodation is the confounder every one of these methods is built to defeat. LogMAR scoring is worth knowing properly too, including how partial lines are credited.
How to prepare for binocular balancing
Practise all three techniques under supervision on patients who respond differently, because the value of understanding the principle only shows up when the standard response does not appear. In parallel, drill the underlying optics with exam-style questions so that the chromatic aberration behind duochrome and the fogging logic behind Humphriss are things you understand rather than things you recite.
