Subjective refraction is where the objective result you obtained by retinoscopy gets refined against what the patient actually reports. The technique is a practical skill and there is no substitute for supervised repetition on real patients. What subjective refraction involves
You start from the retinoscopy finding and refine it, working to the best vision sphere, then using the cross-cylinder to settle the cylinder axis before its power, then returning to the sphere once the cylinder has changed. The result is recorded as a clear prescription with the acuity achieved. The sequence matters, and so does knowing why each step comes where it does rather than following it as a memorised routine.
The optics to be fluent in
Know why fogging controls accommodation and what happens to the retinal image when you over-fog or under-fog. Know why axis is refined before power, and what the cross-cylinder is actually asking the patient to compare. Be quick with the spherical equivalent adjustment, so that when cylinder power changes you can correct the sphere without stopping to think about the direction of the change. Add prescription notation and transposition, because the recording step is where an otherwise correct refraction can still lose you marks.
How to prepare for subjective refraction
Book supervised clinics and refract as many patients as you can arrange, because fluency with the phoropter or trial frame and with instructing a patient comes from nowhere else. Alongside that, drill the conversions with exam-style questions until the sphere adjustment after a cylinder change is reflex rather than calculation. Review each worked explanation for the reasoning rather than the answer, since the same logic reappears at the chair in a different arrangement.
